AIP requires each author to submit the following forms (the links to customized pdf files are placed below).  All paperwork must be submitted together with the final manuscript to Andrew V. Pakhomov. 

Mandatory forms:

(1) Transfer of Copyright form: This transfers the copyright to AIP. A signature of one of the authors of the paper is sufficient. In order to get customized form click here.

(2) Permissions Form: It is necessary to obtain permission from the original author and publisher to reprint any previously published material.  Please use provided below form to request the permissions, if necessary. In the interest of time, you may request permission from the copyright holder(s) via fax. In order to get customized form click here.

Optional forms:

(3) Offprint/Reprint order form: Offprints of individual papers are available for an additional charge as described in the schedule on the form. Please note that AIP cannot accept any offprint orders once the manuscript is in production.  Please submit your order together with the final manuscript. In order to get customized form click here.

LaTeX and MS Word 97 Style Files are available at AIP website.  It is very important to follow the instructions exactly as they are described by AIP.  Click Author Instructions and then click 6 x 9 format.  The details on manuscript preparation can be found here.
